Triple papillary fibroelastoma of the aortic valve asociated to cerebral and coronary embolization

We report a 55-year-old male with a history of transient ischemic attack and a non-Q wave myocardial infarction associated to aortic valve papillary fibroelastomas. Three masses were found and resected with aortic valve sparing. Only one lesion was preoperatively observed by transthoracic echocardiography; nevertheless, transesophageal echocardiogram noticed two extra lesions. Histologic examination led to a final diagnosis of papillary fibrolelastoma of the three masses. The aasociation of triple papillary fibroelastoma with cerebral and coronary embolization constitutes an unusual presentation. Since multiple lesions can be missed on transthoracic echocardiographic evaluation, the transesophageal approach could be recommended when a papillary fibroelastoma is suspected.
